PROGRAMS WE OFFER
DRIVER'S LICENSE
You must provide Driver's License or State issued ID for all household members age 18 or older.
SOCIAL SECURITY CARD
You must provide Social Security Cards or other approved verification (ie: food stamp budget sheet, Medicare card or Income Tax Return) for all household members.
PROOF OF INCOME

Last 4 consecutive check stubs

Current year's award letter for pensions, Social Security or VA Benefits (federal benefits may also be verified using most recent bank statement.

Proof of Child Support or other support including contributions.
UTILITY BILL

Utility bills may not exceed prior to six months

Must include a portion of the bill that shows usage

Utility Account cannot show a credit balance.

If you are seeking assistance for more than one utility vendor (ie: electricity, gas and /or propane), both bills must be from the same month.
PROOF OF HOUSING UTILITY ALLOWANCE
If you reside in a Housing Authority-funded property, you must provide a copy of your utility allowance worksheet residency. (LiHEAP ONLY)
PROOF OF RESIDENCY

This is required if the address on your driver's license or State issued ID does not match the address that is listed on your utility bill.

Acceptable documentation includes pay stubs, award letters, budget sheet, rental/lease agreements, mortgage statement, phone bill, cable bill, rent receipt, proof of insurance or other current mail. Please note: Any documentation used must be dated within the last 30 days.

Provides assistance with utilities, rent/mortgage, prescription drug, must be 60 years or older, meet Federal Income Guidelines and have not been assisted in the last 6 months.

CSBG EMERGENCY ASSISTANCE PROGRAMS
Provides assistance with utilities, rent/mortgage, prescription drugs, must have a disconnect, termination of services or eviction notice, high medical bill and some sort of income.

FOOD FOR FAMILIES/FOOD FOR SENIORS
Seniors receive 40 pounds of food monthly, must be 60 years or older and meet USDA Income Guidelines.

LIHEAP
Provides assistance with utilities must meet Federal Income Guidelines and have not been assisted in the last 6 months.

FEMA
Provides assistance with emergency food purchase, must meet Federal Income Guidelines, demonstrate a need for food and receive little or no food stamps.

LIHEAP CRISIS
Provides assistance with utilities must have disconnect or termination of service notice and have not been assisted in the last 12 months.

VOLUNTEER INCOME TAX ASSISTANCE (VITA)
Free Income Tax preparation with automatic EITC determination and e-filing for clients earning up to $54,000 annually.

PAMPER ME WITH LOVE
Provides diapers for infants and adult incontinence supplies

HiSET PROGRAM
Covers the cost of HiSET testing for students taking/passing the GED Exam.

USDA SURPLUS COMMODITIES
Receive food items quarterly (January, April, July, & October), must meet USDA Income Guidelines.

DHHS MEDICAID AND SNAP APPLICATION ASSISTANCE
Provides assistance with online completion of applications for services provided through the Department of Health & Human Services. These services include Medicaid and SNAP benefits.

HUD COUNSELING
Receive free counseling in the areas of first-time homebuyers, budget/money management, and renter's rights.

ATMOS
Provides assistance for ATMOS Energy customers, must have a disconnect notice, reduction income or an increase in necessary expenses. Priority shall be given to the elderly, families with your children and persons with disabilities.
